- H.S.C. Mark sheet
- H.S.C. Passing/Board Certificate
- Medical Fitness Certificate ( As per the NEET Brochure format )
- EWS or Minority Certificate/Affidavit( if applicable)
- College Leaving Certificate
- Gap Certificate (if applicable)
- Transfer Certificate / Migration Certificate (if applicable)
- Aadhar Card (Attested Xerox)
- Caste Certificate(for reserved category)
- Caste Validity Certificate – (for reserved category) Mandatory
- Non-Creamy Layer Certificate ( If Applicable )
- Photographs Passport size – 5 copies
